We value parent input in developing well-balanced classes, so if you would like to share any pertinent information about your child entering grades 1-5 next year, please use the following form.  You may submit a form for each child that you have in our schools.  This form is completely optional and it is important to know that we do not take requests for specific teachers and/or to be placed with specific students.  Submissions received after May 5th may not be considered.  Thank you for your input!
