2022-23 COVID Testing Consent Form

Only complete this form if you are giving the school permission to test your child for COVID-19.

What is this form?

We are seeking your consent to conduct point of care testing on your child during the 2022-23 school year. COVID testing in school is a parent or guardian choice and is not mandated

When would my child be tested?

A parent or guardian will always be contacted before a child is tested even with a signed consent. 

School testing can be utilized if a person is presenting symptoms.  This would help the parent determine if the child has the COVID virus. 

Or

If your child was a close contact and you want them tested.  This is completely optional.

What is the test?

If you consent, your child will receive a free diagnostic test for the COVID-19 virus . Collecting a specimen for testing involves inserting a small swab, similar to a Q-Tip, into the front of the nose. 

The district has access to two different types of COVID tests:  1.) A molecular test - the sample is processed using an Abbott testing machine  2.) Antigen Test - This is the traditional method that can be taken at home and purchased at a pharmacy.

What should I do when I receive my child's test

Test results will be available to school staff within 15 minutes of collection. You will only be contacted if your child is positive.  If someone tests positive the person should isolate for 5 days at home.  As long as symptoms are residing, the child can return back to school on day 6 wearing a well fitted mask until day 10.  If your child's test results are negative, this means the virus was not detected in your child's specimen.
