Registration for Group Yoga Class for May - June 2024 @ The Every Studio (EARLYBIRD ends 26/04/24)
Hello There!

Here's a simple form to sign up for the Term Yoga classes!
Start a routine, dedicate just ONE hour on your body and mind per week.

GROUP YOGA CLASS is Term Based. 
which means it is fixed timing every week, same timing, same place (for the months) that we will see you for class. 

*This Term is for May - June 2024.*
**Early Bird Promo** (EXTENDED) Ends on 26/04/2024 FRIDAY 2359hours
8 CLASSES = $120 (UP: $160)
7 CLASSES = $105 (UP: $140)

Tuesdays (8 classes): 
7th, 14th, 21st, 28th May 
4th, 11th, 18th, 25th June

Wednesdays (7 classes): 
8th, 15th, 29th May (No Class on 1st & 22nd) 
5th , 12th, 19th, 26th June 

Saturdays (8 Classes): 
4th, 11th, 18th, 25th May
8th, 15th, 22nd, 29th June (No Class on 1st)

Sundays (8 Classes) 
5th, 12th, 19th, 26h May
9th, 16th, 23rd, 30th June (No Class on 2nd)
  
(No refunds or cancellation if unable to make it for any class.)

Yoga Class Timings <Each class is 60 mins>
1) Tuesday - Strength & Stretch - 7:30pm 
2) Wednesdays - Power - 7pm
3) Saturdays - Power - 8:45am 
4) Saturdays - Strength & Stretch - 10am 
5) Saturdays - Flow - 1:30pm 
6) Sundays - Power - 9am

We start on time so make sure you come at least 5mins earlier to prep yourself. 
Be in your comfortable sports attire and a small towel (just in case) to wipe your sweat. 
Mats and props will be provided! 

Payment will be by Paynow to our UEN: 53428680A (THE EVERY STUDIO). 
Take a screenshot of your payment and send to us!

**Slots are only confirmed after payment is made.**
**If there are insufficient sign ups, The Every Studio have the right to: 
(1) offer you a switch in class timing OR 
(2) class type OR 
(3) refund you the full payment.**

***Strictly no refund if you are unable to make it for any class.***
Once classes are confirmed, no refund/cancellation is allowed for the classes. 
If you are absent from the class, there is no refund allowed. 

*** 1 MAKE UP CLASS is allowed this term (May & June 2024) if you are unable to make it for the class.***

If you are unable to make it for class, please inform the studio via Whatsapp +6588271149 - (whatsapp link here.) at least 6 hours prior to the start of class in order to qualify for a *make up class.

*Only 1 make up class is allowed this current term (May & Jun 2024)

Liability Waiver  ** Please Read & Acknowledge Below ** *
If at any time during the class, you feel discomfort or strain, gently come out of the posture. You may rest at any time during the class. It is important in yoga that you listen to your body, and respect its limits on any given day. I understand that yoga includes physical movements as well as an opportunity for relaxation, stress re-education and relief of muscular tension. As is the case with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. If I experience any pain or discomfort, I will listen to my body, discontinue the activity, and ask for support from the instructor. I will continue to breathe smoothly. I assume full responsibility for any and all damages, which may incur through participation. I understand that Yoga is not a substitute for medical attention, examination, diagnosis or treatment. Yoga is not recommended and is not safe under certain medical conditions. 

By signing, I affirm that a licensed physician has verified my good health and physical condition to participate in such a fitness program, if required. In addition, I will make sure the instructor is aware of any of my medical conditions or physical limitations before class. If I am pregnant, become pregnant or am post-natal or post-surgical, my NAME/INITIAL BELOW verifies that I have my physician’s approval to participate. I also affirm that I alone am responsible to decide whether to practice yoga and participation is at my own risk. I hereby agree to irrevocably release and waive any claims that I have now or may have hereafter against The Every Studio and all related facilities and premises for any personal injury or negligence. Additionally, the facility, instructor and The Every Studio are not in any way responsible for any loss or damage of your personal property. Those under 18 years of age must have this form signed by a parent or guardian. If any portion of this release from liability shall be deemed by a Court of competent jurisdiction to be invalid, then the remainder of this release from liability shall remain in full force and effect and the offending provision or provisions severed here from. 

I have carefully read and fully understand and agree to the above terms of this Liability Waiver Agreement. I am signing this agreement voluntarily and recognize that my NAME/INITIALS BELOW serves as complete and unconditional release of all liability to the greatest extent allowed by law and that it cannot be changed orally.
