In order to ensure that the church is adequately prepared to receive guests and properly sanitize the facilities for events please complete the following form. In order to adhere to social distancing guidelines all events will require that you preregister/pre-identify your attendees.

Please note that face masks, disclosure forms and temperature checks will be required for all participants.

You will receive a response from the New Life Baptist Church Board regarding your request. Please note that at this time we are only accepting requests for internal church events.
