Upon parent/guardian request ONLY, SNS will be providing allergy free meals for students during the COVID-19 curbside meal service.
These meals are free from the Big 8 allergens: Eggs, Peanuts, Tree Nuts, Fish, Shellfish, Wheat and Soy.
Monday and Wednesday Pickup: Two breakfasts and two lunches will be provided.
Friday Pickup: One breakfast and one lunch will be provided.
PARENTS/GUARDIANS MUST PICK UP MEALS FROM THE SITE IDENTIFIED AND APPROVED VIA THIS FORM.
Once the form is approved, parents/guardians will receive an approval letter from SNS via email. Form will be approved within 2 business days.
Day of Meal Pickup:
1. Show approval letter to SNS curbside staff.
2. SNS curbside staff will notate the number of children approved for allergy free meals on the SNS car tag.
3. Parent/guardian will place car tag in the passenger windshield for future meal pickups.
