Waiver CUERELEASE AND WAIVER OF LIABILITY AND ASSUMPTION OF RISK This document is being given regarding the undersigned's voluntary decision to participate to in educational opportunities/activities sponsored by Communities United in Education of Siren and Webster, Inc. The undersigned acknowledges and agrees that there are innumerable dangers and risks involved in the educational opportunities/activities being provided, and that no amount of care, caution and/or instruction can eliminate these dangers and risks. Despite these dangers and risks, the undersigned desires to participate in these educational opportunities/activities and that all the undersigned's questions have been answered to his/her satisfaction.In consideration of being permitted to participate in the educational opportunities/activities being offered by Communities United in Education of Siren and Webster, Inc., the undersigned, for himself/herself, his/her personal representatives, heirs, and assigns:1. Acknowledges, agrees, and represents that he/she has or will exercise prudence and caution and if he/she feels anything to be unsafe will immediately ask to stop participating in any opportunity/activity they are performing.2. Hereby releases, waives, discharges and covenants not to sue the officers, directors, volunteers, employees, or independent contractors of Communities United in Education of Siren and Webster, Inc., from all liability to the undersigned, his/her personal representative, assigns, heirs or next of kin, for any and all loss or damage and any claim or demands therefore, on account of the undersigned participating in any opportunity/activity, whether caused by the negligence of the releasees or 3. Hereby assumes full responsibility for any risk of bodily injury, death or property damage, arising out of or related to, participating in any opportunity/activity offered by Communities United in Education of Siren and Webster, Inc., whether caused by the negligence of releases or otherwise.4. Hereby acknowledges that the opportunities/activities offered can involve a dangerous activity and can involve the risk of serious injury and/or death, and expressly acknowledges that injuries received may be compounded or increased by negligent rescue operations or procedures of the releasees.5. Hereby agrees that this release and waiver of liability and assumption of risk agreement, extends to all acts of negligence by the releases related to the participation in opportunities/activities offered by Communities United in Education of Siren and Webster, Inc., and it is intended to be as broad andinclusive as is permitted by the laws of the State of Wisconsin in which the events are conducted and that if any portion thereof is held invalid. it is agreed that the balance shall, notwithstanding, continue in full force and effect.IN SIGNING THIS RELEASE AND WAIVER OF LIABILITY, I UNDERSTAND THAT I AM GIVING UP IMPORTANT LEGAL RIGHTS THAT I MAY OTHERWISE HAVE.I HAVE READ THIS RELEASE AND WAIVER OF LABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND IS TERMS. UNDERSTAND THATHAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING I AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE OR GUARANTY BEING MADE TO ME AND INTEND MY SIGNATURE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW. *
