D. DECLARATION
In consideration of Epsom College in Malaysia (“ECiM” or “the College”) agreeing to allow my child to participate in this programme, I agree to the following.
a. I am aware that the programme involves physical activities and I am responsible to take the necessary insurance coverage.
b. I acknowledge that I am required to disclose all pre-existing medical conditions of my child via the online Student Medical Form (to be provided by ECiM) in order for the College to provide the best possible care to my child during the programme.
c. I agree that my child is expected to enter fully into the life of the College and to take part in all the programme activities. He/she is subject to the same school rules and disciplinary procedures as the College pupils as per the terms and conditions of this programme.
d. I will not hold ECiM, including teachers and other staff, liable for any loss or damage suffered by my child during the programme unless the same is caused by negligence or default of an ECiM representative.
나의 자녀가 말레이시아 엡솜컬리지 프로그램의 참여할 수 있도록 다음 사항에 동의 합니다.
a. 나는 엠솜컬리지 영어캠프 프로그램에 신체적 활동이 포함되어 있음을 알고 있으며, 이에 필요한 보험혜택을 받을 수 있는 보험을 개별적으로 가입할 책임이 있습니다.
b. 나는 학교가 프로그램 기간동안 자녀에게 최상의 치료를 제공할 수 있도록 엡솜컬리지에서 제공한 온라인 학생 의료양식 폼(online Student Medical Form)을 통해 자녀의 건강 상태를 모두 공개해야 함을 동의합니다.
c. 나는 자녀가 학교 생활에 완전히 참여하고 모든 프로그램 활동에 참여해야 한다는데 동의합니다. 학생은 캠프기간 엡솜컬리지 재학생과 동일한 학교 규칙 및 징계 절차를 따릅니다.
d. 엡솜컬리지 담당자의 부주의(negligence ) 또는 불이행(default )으로 발생한 경우가 아니라면, 교사 및 직원을 포함한 엡솜컬리지 말레이시아는 프로그램 중에 내 자녀가 입은 손해에 대해 책임을 지지 않습니다.
