All of the undersigned agree to waive all rights to sue or maintain suit whether in law or in equity for any injuries that may occur while serving as a volunteer of the 5K Turkey Trot for Charity.  By signing this form, you acknowledge that you understand the content of this covenant not to sue and that you agree to be bound to the terms and conditions described herein.  Signing this form implies permission to allow your image(s) to be used by Samoan Solutions for promotional purposes.  People under the age of 18 must have a parent or legal guardian to sign this form.
