Your purchase of carnations is considered your entry fee. Carnations are to be purchased on our website before submitting this form. Carnations are $7.00 per dozen. Car clubs must purchase 2 dozen for each vehicle, floats are required to purchase a minimum of 5 dozen (to be included in the design of the float). All other units must purchase a minimum of 3 dozen carnations. My carnation order number: *

In the case that your unit wins an award or recognition, those awards will need to be picked up from the Alliance Chamber of Commerce the week following the parade. *

I acknowledge, that it is my responsibility as the contact person to pick up these awards or have another representative from my organization pick up awards.

Please take notice that this is a public event and photographs may be taken. The Greater Alliance Carnation Festival does not guarantee the subject(s) in the photographs will be contacted in the event the photos are published. Accordingly, by attending the event, you acknowledge and agree that the Greater Alliance Carnation Festival shall have the right (without limitation or compensation to you) to take, edit, adapt, modify, reproduce, publish, promote, display and otherwise use your photograph in any way it sees fit, including the right to publish your photographs online and in print. *
