In order to follow protocol to protect privacy and ensure informed consent, JFS Houston will collect your contact information (full name and email address). This information will not be shared publicly.

If you decide not to participate in providing a testimonial, it will in no way impact your ability to receive services from JFS Houston.
 
Use of Testimonial and Consent
I understand my testimonial may be shared on social media, printed publications, multimedia presentations, jfshouston.org (JFS Houston's website), and other marketing materials. I authorize JFS Houston to copy, exhibit, publish or distribute the Testimonial for purposes of publicizing JFS Houston's programs or for any other purpose. I agree that I will make no monetary or other claim against The Business for the use of the statement.

JFS Houston reserves the right to edit language for privacy purposes and clarity. I waive the right to inspect or approve the written copy wherein your testimonial appears.

I hereby hold harmless and release JFS Houston from all claims, demands and causes of action which I, my heirs, representatives, executors, administrators or any other persons acting on my
behalf or on behalf of my estate have or may have by reason of this authorization.
