The National Association of Guide Dog Users, a division of the National Federation of the Blind, would like to hear from you if you have been denied a ride on the Lyft rideshare service because you are traveling with your guide dog or other service animal. In addition to using NFB’s Rideshare Discrimination Survey, https://www.nfb.org/rideshare, to gather information, during the week of January 24-30, 2022, we will be gathering your experiences in video form to present to Lyft. You do not have to be a member of the National Federation of the Blind to complete this survey or submit a video.
We ask that the videos are filmed after a denial occurs, not during. Videos should be no longer than one minute in length and should describe who you are and the impact of the denial. I.E. did you miss a doctor’s appointment or were you late to pick up your child from daycare?
By submitting a video, you are giving us permission to share your information with Lyft to make them aware that blind people with legitimate guide dogs and other service animals are still getting denied.

IMPORTANT! The survey responses and videos we are collecting are for Lyft denials which happen during the week of January 24-30, 2022. This form will be open until February 6, 2022 to give everyone up to a week to submit their responses and videos.
