Please complete the following to give West Ryde Before and After School Care Inc. permission to take your child to/from their extra curricular activities
At West Ryde BASC Inc. we
understand that some mornings or afternoons children may need to go to an
extra-curricular activity, such as soccer, chess or orchestra etc. If your
child needs to attend an extra-curricular activity that is going to impact on their
time in our care at the centre, whether it be they are late to after care or
need to leave early from before care, the must be informed in writing prior to
this occurring. This is to ensure the safety of all children in our care. If
the extra-curricular activity is located on school grounds, staff members are
able to walk your child/ren over to the activity with your written permission.
Please fill out the below form if your child needs to attend any
extra-curricular activities.
