Your identity will be kept anonymous to everybody beyond the equity team and executive director unless you consent to us telling them. Our goal is to ensure that we address all equity issues in the NYPDL while also preserving anonymity whenever it is possible. If you are reporting cheating, your identity will always be 100% anonymous to everybody beyond the equity team and executive director.

REMINDER: this form is not a place to complain about losing a round but rather a place to report genuine equity violations. A bad judge is (usually) not an equity violation, it's just annoying. We trust you to know the difference between actual equity violations and being upset you lost a round, and to not abuse this form.
