The undersigned is voluntarily agreeing to participate in Team Illinois Lacrosse Events for his own benefit and/or enjoyment. In consideration for permission to participate in the Invitational, the undersigned acknowledges and agrees as follows:

1. I acknowledge that the risk of injury from participation in the sport of lacrosse is inherent and significant, including the potential for permanent injury.

2. I knowingly, freely and voluntarily assume all risks, both known and unknown, in connection with my participation in lacrosse, including risks arising from the negligence of other participants in the sport; organizers, coaching staff and/or their agents; and all others acting on behalf of or through the direction of the organizers and/or coaches. 

3. I further agree that if I observe any unusual significant hazard during my participation in the sport, I will remove myself from exposure to said hazard and will immediately bring the hazard to the attention of any coach or other authority figure on the premises.

4. I, for myself and my heirs, agents, personal representatives, administrators, next of kin, successors and assigns, hereby forever waive, release and discharge the organizers of the Club, coaches and agents of and from any and all claims, actions, causes of action, damages or other relief, including without implied limitation, injuries, disability, death, loss or damage to person or property or other liabilities whatsoever arising from or relating in any way to my participation in this club sport, whether arising from the negligence of organizers, coaches , other team participants, or otherwise to the fullest extent permitted by law.

5. I, for myself and my heirs, agents, personal representatives, administrators, next of kin, successors and assigns, hereby covenant and agree to indemnify and hold harmless organizers, coaches, other team participants of and from any and all claims, actions, causes of action, damages or other relief, including, without implied limitation, injuries, disability, death, loss or damage to person or property or other liabilities whatsoever arising from or relating in any way to my participation in lacrosse whether arising from negligence or otherwise, to the fullest extent permitted by law.

I represent and warrant that I am at least eighteen (18) years of age and that I have read this Waiver and Release of Liability, that I understand its terms, that I understand that I have given up substantial rights by signing it, and that I freely and voluntarily sign it. If not yet eighteen (18) years of age, I will sign and my parents or legal guardian will also sign on my behalf and agree to be bound by the terms of this Waiver and Release of Liability.
