Please fill out a form for EACH of your cheese orders. This will help us keep track of all the orders coming in. You are responsible for collecting all the money for the orders that you place. All orders are to be placed by November 23rd - no more orders will be taken after that date. All order money will be collected on November 30th.

Each sampler box of cheese sells for $20. The cheese should arrive by Dec. 14.
