COMPLETE THIS FORM ONLY IF YOU ARE REQUESTING A CHANGE for your student(s) in KINDERGARTEN through 6TH GRADE.  Do not fill out this form if you want your child to remain in their current learning environment.

Please complete this form by TUESDAY, NOVEMBER 24th.   If you want to request a change for more than one child, you will need to complete one form for EACH child.  Families who request this change should look for additional communication from school administration during the month of December.

Changes will be effective with the start of second semester, on January 4, 2021.
