Clear Path Paper National Teacher Appreciation Day 2023 Giveaway Entry Form
Fill out the form below to be entered into our National Teacher Appreciation Day giveaway! You must enter by 11:59pm EDT on Wednesday, May 3, 2023 in order to be eligible to receive a scrap pack. We will fulfill all requests on a first-come, first-served basis while supplies last. Any individual who is nominated more than once will not receive more than one (1) scrap pack. If two individuals from the same school are nominated, then no more than one (1) pack will be sent. We will do our best to fulfill as many requests as possible, but fulfillment is not guaranteed. We reserve the right to refuse to fulfill any request, especially where school policy prohibits direct donations to educators. US only, please.
