(Intention Survey for Physical Examination reservation of Overseas Freshman)

■體檢方案(請二擇一):Types of health-check (please choose one of them)

方案一：新生體檢  
Type A.　Only NCKU Freshman request items 
(NT$850)(discount period) / (NT$1200)(without discount period)

方案二：新生體檢 + 簽證用體檢  
Type B.　NCKU Freshman request items + for changing visa or ARC 
(NT$1300)(discount period) / (NT$1680)(without discount period)

■體檢流程 (Instruction) ：

(Read over the information on website first --> arrive Taiwan --> complete this google form --> NCKU health center will send the voucher to your email box  ---> show the voucher to the hospital counter for register --> After health checking, bring the pink receipt of NCKU hospital to the Health Center in school for stamping)

■其他入學前需下載本校防疫app-(Besides the freshman physical exam, this app is needed to be download.)
