Phi Beta Sigma Fraternity Inc. Delta Omicron Sigma Chapter in conjunction with the DOS Crescent Foundation Adopt A School Funding Request Form
PURPOSE: The Adopt-­A-­School component of Phi Beta Sigma Fraternity Inc. is designed to get Sigma men into the schools and programs in their community. Our effort is to brand this component of Sigma’s program as value added activities, to build student literacy and accrue educational service hours and presence  in schools. While in the schools, the local Phi Beta Sigma Fraternity Inc. chapter (Delta Omicron Sigma) and the DOS Crescent Foundation Inc. 501c3 of the Delta Omicron Sigma Chapter will implement or assist in one or more of the Sub-components listed:

Reading Days
Essay Contests
Spelling bees
Oratory and Debate Competition
Academic and Positive Reinforcement Events
Math and Problem Solving Competition
STEM related Experiences, Field Trips, and Projects
Community Events
Family Nights

FUNDS:  Funding is intended to supplement components’ and affiliates’. These funds should act as seed money for new events with the goal that the affiliate or component will find success and include it in their regular budget. Funding is intended for future events (events that have already taken place will not be considered). Funding in the amount of $100 per year will be allocated for each site to be used for educational events and activities. Once funding is approved, pictures of the event must be sent to provide documentation that the event was completed.

REQUEST FOR FUNDS: Submitting the following online request form must be completed at least (1) months prior to event for approval.  

FUNDING PRIORITIES: The program established that funding priority will be given to those component and affiliate societies holding unique events that engage target groups.  The funds are not intended to replace regularly budgeted events or events that have been funded in the past.  

EVENT DETAILS: Special events defined under Project Colleague would include, but not limited to, receptions, luncheons and dinners, and continuing education lectures.   The component should be notified of the final event details to help increase awareness of the event at the district level.

METRICS FOR SUCCESS: Please keep a record of each attendee and submit the total attendance along with pictures for documentation purposes. Funding will not be granted in the future to entities that have failed to provide their reporting forms.  The reporting info is due (2) weeks after each event.
