To participate in this group, you must agree and abide by the conditions stated below.

1. I acknowledge and agree that involvement in the consultation group is in no way a substitute for the
supervision of cases.
2. I understand that I am responsible for individually and independently satisfying myself of the soundness of
any suggestions, recommendations, or advice-like comments made by the Mego Nerses.
3. I understand and agree not to hold Mego Nerses liable for any advice/comments he provides to me.
4. I agree that everything I hear and share in the group will be confidential, and I will not share it with anyone
else.
5. I understand and agree to make the proper payment to Mego Nerses immediately prior to the consultation
session. Otherwise, I will not be able to join the meeting. Receipts are available to me if needed.
6. I understand and agree that to reserve my spot; I need to make the payment before the session.
7. I understand and agree that Mego Nerses will send me the weblink to the online meeting by email two days
before the meeting.
8. I understand that I may be asked to leave the meeting/program if I disrespect any participants or
demonstrate unprofessional conduct. I consent that Mego Nerses has sole authority to make such
determinations.
9. I understand and agree that Mego Nerses abides by the laws related to confidentiality and limits to
confidentiality. He has a duty to report in case if he has reasonable grounds of unethical and unprofessional
conduct with clients (harmful to clients).
10. I understand and agree that refunds are not available at this time.
11. I agree and consent that Mego Nerses register my name, phone number, email address, addresses in the
Jane App for accounting and charting purposes.
12. By submitting this application, I confirm that I agree to abide by all the above guidelines and statements.
