This form is for RETURNING STUDENTS ONLY. If adult student, please visit our Adult Private Lesson Info Form online. If you are a NEW Student, please fill out a NEW STUDENT INQUIRY.

If you are a registering RETURNING child student, please fill out this form to sign up for Fall 2022 private lessons. You will then receive an invoice within 72 hours. Once this invoice is paid or a payment plan is set up, your registration will be complete. To pay using a payment plan, you MUST have a credit card on file with us.

Regular Registration: August 15-22
Late Registration (with $25 late fee): August 23-28

The Summer session runs from August 29, 2022 to January 15, 2023. The minimum number of lessons is 15. A full slate of lessons is 17. For more detailed scheduling information, please see our School Calendar.

I understand that most teachers are offering on-site lessons at this time, while some are offering remote only or a mix of both. My teacher will communicate my options with me directly. I am responsible for paying for all lessons with my teacher. (enter initials to sign) *

I understand that this is just a registration form, and that registration will not be complete until payment is received. I will be invoiced within 72 hours of submitting this form.  I understand that payment (either in full or a payment plan by cc) is required PRIOR to the first lesson of the session. If payment is not received in full or if I do not set up a payment plan by the first lesson, I understand that my lessons will be put on hold until payment is received. Photo Release: I grant The Musical Offering permission to use photos or videos of my child related to Musical Offering events or classes for legal use, including but not limited to: publicity, advertising, social media, and web content *
