Access to the FIS is restricted to CWRU faculty and administrative staff involved in business processes involving faculty information.  Those interested in obtaining access to the FIS complete and submit the form below to begin the authorization process.  Your request will be presented to the appropriate dean/executive office for approval.  Once the approval is obtained, your role at the institution will determine the level of access to faculty information you will be assigned in the system.
