Important reminders:

• Food and refreshments (samosas, pizza, snacks, gatorade, water) will be provided.
• Complete this form and submit the participation fee ($30/person) by Wednesday, March 1st.
• Participation fees can be - submitted by Zelle (account: controller@sewausa.org) or Venmo (account: @sewainternational). Mention "ping pong tournament" in memo.
  
• Sewa International USA and its members are not responsible for any injuries or accidents.
• Ages 14+, singles (not doubles)
  
• Bring your own paddles if possible. Limited number of paddles will be available otherwise.
