The Tulip Award is given to the staff member who on a monthly basis, makes a meaningful difference to our patients and family members and others that we serve. Nominations may be accepted from our staff, patients, family members, visitors, and other external customers.

The Tulip Award honors employees who model compassion first, Integrity always, Respect for all, and Excellence in everything.

Who can be nominated?

RN’s, LPN’s, Physical Therapists, Occupational Therapist, Speech Therapist, Aides, MSW’s, Spiritual Care Team and office support staff who you feel deserves recognition may be nominated for a TULIP Award.

Who can nominate?

Any patient, family member or visitor in your home is welcome to nominate a deserving staff member by completing this form and returning it to a VNA and Hospice of the Wabash Valley representative.
