I agree to abide by the terms of any adoption contract between myself and Underdog Pet Rescue and Rehabilitation for the adoption of any animal. I agree to the terms of ownership that includes standards of care, including, but not limited to: keeping my cat indoors, agreeing to never declaw, and to follow any discussed behavioral plans. I understand that in the event that I can no longer care for this pet that I will contact Underdog Pet Rescue and Rehabilitation before rehoming, surrendering, or euthanizing this pet for behavior. In some rare circumstances, Underdog may opt to perform a home consultation prior to placement of the pet to help prepare for the transition. I consent to a home consultation` visit. Underdog also provides in-home services to help get pets settled and create plans for introducing to any existing pets. I understand that this is available to me and will utilize these services if needed.I agree to provide a high quality of life for this pet including but not limited to appropriate housing, vet care, and safety. Should I be unable to provide this quality of life, I agree to contact Underdog Pet Rescue and Rehabilitation for help; if we are unable to reach a solution, my pet may be returned to Underdog’s care.
