Please indicate whether your child will go to the Kid's Programme or stay in church with you. Children will go straight to their programme from the beginning and not join the adults in the auditorium.

You can complete a form for another person in your family - there will be a link at the end to "submit another response".

Should you or members of your household develop symptoms or be in contact with someone with COVID in between completing this form and the upcoming Sunday, please do stay at home, for the safety of the others attending.

BY BOOKING A SEAT FOR OUR IN-PERSON CHURCH SERVICE YOU UNDERSTAND AND AGREE TO THE FOLLOWING:

I am currently NOT experiencing any of the following symptoms: fever, cough, sore throat, shortness of breath, difficulty breathing, body aches, loss of smell or taste, nausea, vomiting, diarrhoea, fatigue, weakness.

If I experience any of the above symptoms on the day I have booked in for, I will not attend the service.

I agree to wear a face mask at all times covering both my nose and mouth.

I agree to have my hands sanitised upon arrival.

I agree to adhere to the social distancing practices stipulated by the Health Department of South Africa and the church.

I agree to the church recording my personal contact details for the purposes of ‘contact tracing’ should the need arise.

I agree that if I have been exposed to Covid-19 I will not attend services within 14 days of said exposure.

I agree that if I have tested positive for COVID -19, I will immediately inform any church leader if I have been at the church in the 14 days prior to being tested positive.
