I hereby give my permission and approval for my child to participate in the Bellingham AYFC Kickball Tournament. I assume all risks and such hazards incidental to such participation, and I hereby release Bellingham AYFC and Bellingham Public Schools from any and all liability arising from injury or injuries sustained by my child while participating in the Kickball Tournament. Neither Bellingham AYFC nor Bellingham Public Schools assume responsibility for any damage to or loss of any personal or team property.

I hereby authorize the volunteers of Bellingham AYFC to obtain medical care for injuries and illness that might affect my child or which might occur during the tournament. I further direct all medical or hospital facilities to accept this document as authorization to render emergency care to my child should it be deemed medically necessary.

I certify that my child has not had close contact with a person confirmed to have COVID-19. I certify that my child does not currently, and has not had in the past 24 hours, any of the outlined COVID-19 symptoms. My child does not have a fever above 100° F / 38° C and has not felt feverish in the past 24 hours.

Divisions:

DIVISIONS: Teams will be assigned to a division based on the ages/grade level of team members. All team members must fall within the grade brackets: Grades K – 2, Grades 3 – 5, Grades 6 – 8, Grades 9+.

Rules

All players must have a signed Liability Release form to play (see above). Teams must have 8 players – Bellingham AYFC reserves the right to assign “free agents” to any team. No swapping players from team to team or both teams will be disqualified. No trash talking, foul language, taunting or physical contact with other players. Please be on time for all games; if a team does not show up after 3 calls, the team forfeits that game. Game durations will be determined by the number of teams registered. World Kickball Association rules will be followed and may be viewed here: https://kickball.com/rules/. Referee calls are final. Arguing or harassing referees will result in team disqualification.
