This is the registration for school year 2021-2022 care.

Care will be offered M-F from 6:30 a.m to 7:30 a.m. and after school until 6:00 p.m on days school is in session, Thursday, September 9th, 2021 to Friday, June 3rd 2021.

ENROLLMENT OPTIONS:

BEFORE SCHOOL OPTION:
School Year AM--------- 6:30-7:30 am: 5 days per week, $28
7:30 am students are released to outside and/or to breakfast until 8 am bell
Includes care on Wednesday Late Start AMs from 6:30-8:30 am.

AFTER SCHOOL OPTION:
School Year PM--------- 2:50-6 pm: 4 days per week, $40 OR 5 days per week, $50

Drop-in is only available to families with 4 day contracts, and as space and adequate staffing permits; call to check on availability. Fees will be applied.

Enrollment Process:
Registration/Approval Process Limited Enrollment
Eagles Nest SACC may find it necessary to limit enrollment due to potential space limitations, staff-to-child-ratio requirements, and/or other constraints including guidance from state, federal and district requirements and protocols.

Serves:
School Year Program serves children who are enrolled in Gr. K-5.
This means children can start attending Eagles Nest on the first full day of Kindergarten and can remain enrolled until the last day of school during their 5th grade year.

Lottery: If an enrollment maximum has been reached, students will be placed on a list and selected via lottery.

HOW TO REGISTER:
This is the form to register for School Year care.

1. Complete this Google doc on-line Registration Form. Registration is open from Monday May 17th through Friday May 21st or until the program fills. We will use the lottery to determine who gets into care when it is over-enrolled. Email confirmations will be sent out after lottery.

2. After you receive confirmation, additional registration documents will be sent to you via email to complete and must be completed and submitted to SACC Coordinator within three business days.

3. Registration payments must be received at the district office by June 1st to secure a space. Details regarding payments will be emailed to you with confirmation.

Ongoing Modifications to policies regarding care, program size, and availability may occur.

Group child care carries inherent risks because of its nature as a congregate activity. By signing up for School Age Child Care, I understand I am accepting the risks inherent in this activity and releasing Dover-Eyota Public Schools from all claims of injury that result from participation in care. Children must be well enough to attend care. Guidelines regarding when students may attend or are excluded from care are provided by Public Health and include both the child's symptoms and exposures to COVID19. It is absolutely imperative to stay home when experiencing COVID symptoms (e.g., fever, cough, chills, nausea, etc.) and while awaiting test results. Type your name here stating that you will only send your child when they are healthy per our guidelines.
