Transportation Change Form- Submit by 2PM
If your child needs to have a temporary change to their daily method of transportation, complete and submit this form or send a signed note to school with them prior to the change. If your student needs a more permanent change to their transportation method, please contact the school directly. 
All transportation change requests need to be submitted no later than 2:00pm on a regular school day in order to be accepted.  If you submit anytime after 2PM you MUST call the office.
