Student COVID-19 Testing Consent Form
COVID-19 testing will be strictly voluntary and can only be performed at the request of a Parent/Guardian. Under no circumstances will a student be required to receive a COVID-19 test.
Drive through testing will be done from 9am-2pm, Monday-Friday at Will Rogers Elementary. After completing this form, please call 580-562-4851 to schedule an appointment.
 
●As needed, our school nurse at BFDC is now able to safely and effectively conduct COVID-19 testing.
●Testing will be available, by appointment, to any students requesting a test and showing no symptoms.
●Parents/Guardians will need to authorize their student(s) to be tested for COVID-19.
●All test kits will be provided and approved for use by the Oklahoma State Department of Health.
