Thank you for helping your child to participate in our pup naming contest for Mexican gray wolves. We are sending you this parent/guardian consent form to both inform you and to request permission for your child’s artwork/essay and personally identifiable information to be published on our website (mexicanwolves.org), Facebook page (www.facebook.com/mexicangraywolves), Instagram page (www.instagram.com/lobosofthesouthwest), and Twitter page (https://twitter.com/MexicanWolves). Your permission and this completed form is required for your child to participate.

We would like to publicize all of the entries, including essays and artwork on our website, Facebook page, Instagram page, and Twitter page. With your permission, we may also display artwork and essays in future press materials and at public events. Please fill out the consent form below.

We would also like to send out press releases in the winners’ communities, and to regional and possibly national news sources. We would like to highlight the winning children and their entries in the press releases. Please answer yes or no for all items below:

Entries must include YOUR CHILD'S FULL NAME and

For Kindergarten through Second grade
* Photographed or scanned artwork (drawing, painting, or diorama) featuring Mexican gray wolves and a brief explanation of why the name was chosen.

For Third through Fifth grade:
* Photographed or scanned artwork (drawing, painting, or diorama) focused on Mexican gray wolves and a brief explanation of why the name was chosen OR a poem or short essay explaining why the name was chosen. You may submit both artwork and a written entry.

For Sixth through Eighth grade:
* Poem or essay explaining why the name was chosen. You may also submit artwork. 

Please note that we are not able to accept physical submissions. All submissions must be electronically submitted via email. Upon request, we can provide a Dropbox or Google Drive link to add large batches of files to support teachers who may be submitting a larger number of entries all at once.
