MEDICATION - List camper’s medication, and frequency/dosage/time of day details if applicable. Medications will be secured and distributed by our church leaders. Only send medication in original packaging/bottles so we can clearly identify them properly.
TRANSPORTATION -  Please note that the church van, rental vehicles, and personal vehicles may all be used for the transportation of groups.  

The above listed camper has my permission to be transported by church staff and volunteers for church events. I understand that my child will be under adult supervision. I further understand that in digitally signing this form, I release and hold harmless Good News Church, its trustees, officers, employees, interns, and volunteers from any liability, past or future, fully and completely. I authorize the staff or designated medical professionals and/or volunteers to administer emergency medical
assistance if I cannot be reached.
