為了更進一步了解家長對貴子女在實驗中文學校過去一年的遠距教學環境與課程的意見，提供老師教學回饋，包括相關教學改進建議，以及對優良教學方法、過程與成果之正面肯定，請家長們以客觀立場並以尊重及負責之態度提出建言，詳細填寫教學意見調查表。填答方式請依據各問題題項，選擇最適合的選項 (請注意同意度順序)，開放題部份，則請填入對老師之課程安排與教學實施之文字意見。
