Food BFFs and Cool Creatures Custom Form
THANK YOU for participating in our Food Besties and Cool Creatures Custom Stocking!  These embroideries were chosen and voted for by you!  The limit for this stocking is 6 embroidered diapers.
This form must be filled out by Sunday, January  15th  at 11 pm CT!   IF YOU DO NOT FILL OUT YOUR FORMS BY THE TIME WE ARE READY TO PRINT CUSTOM TICKETS, WE WILL DESIGN YOUR DIAPER FOR YOU!

We have a good amount of all of the wing options, but if we max out on any wing prints, we will pull them from the form.  Some bodies may be more limited.  If something runs out, we will pull it from the form and honor any over-sales of body requests in order of INVOICE NUMBER.  TAKE YOUR TIME FILLING OUT THE FORM AS ONCE IT IS SUBMITTED, NO CHANGES MAY BE MADE!  Thank you so much and happy designing!  

Embroidery disclaimer: Minor imperfections in embroidery stitching are very normal.  You may see some stitching fall outside of the border, slight gaps between border and stitching or stitches that are not completely smooth.  There may be some puckering around the embroidery where it doesn't lay flat. Breakage of the fabric is also to be expected to some extent.  It is impossible to avoid completely when embroidering stretchy laminated fabric.  To keep your embroideries looking nice and new, use FRAY CHECK at the borders where you see any little tears in the fabric (breakage) to prevent runs.  We try to pull out any with more noticeable flaws and issue discounts for 2nds, but even what passes as first quality may not always be perfect as it is just the nature of embroidery on PUL.
If you are sending in PUL to be used for wings, it must be NO SMALLER than 12" x 12" even for a single wing.  Larger is better, especially if needing a set of wings that have specific cut placement.  You must have a note inside the package with your name and a description or name of the print so I can clip your name to the fabric.  If you send multiple prints, there must be one slip of paper per print.  When you fill out the form, the name/description must match the name on the slip with the words 'sent in'.

What print or solid diaper 'body' would you like?  Some are getting low. If we don't have enough of a certain size/color, they will be awarded in order of invoice/order number.  If we over-sell, you will be contacted.  Colors marked 'old generation' are not as durable/heavy as our new gen colors.  OG prints may have slight print inconsistencies.  IF YOU WON A LIMITED BODY GT AND WANT TO USE IT, WRITE IT IN THE 'OTHER' OPTION.
