Thank you for your interest in this study! Please complete the short survey below if you are interested in participating in an interview. I am seeking two categories of participants: people who have received pelvic physical therapy and pelvic physical therapists. People of all genders and pelvic anatomies are welcome.
The screening tool will take approximately 5 minutes to complete, and will collect information on your gender identity, racial and ethnic identity, age, some brief information about your experiences with pelvic PT, and your contact information. This form will help me determine whether you are eligible for this study. After you submit the survey, I will reach out to you via the email you provide to set up a 1-1 interview. (This may take several months, but if you do not hear from me feel free to follow up by email.)
Your participation in this screening survey is completely voluntary. You are free not to participate or to withdraw at any time, for whatever reason. No matter what decision you make, there will be no penalty or loss of benefit to which you are entitled. Participants will not be paid for completing the screening survey.
