We want to thank you all for being so supportive and understanding during these times. The following guidelines are implemented to ensure the safety and well being of our staff and students. Please fill out the information below prior to entering Linsey's Cheer & Dance Center. Due to COVID-19 pandemic we have altered our service to the public for our 2020/2021 season for both cheer and dance. For the health and safety of our staff, students and families, it is crucial that every person that enters our building follow each of the following protocols.

CLASS CAPACITY: limited availability in each class allowing for the proper social distance. WEAR A MASK AT ALL TIMES: effective immediately staff, students and visitors are required to wear a mask properly inside the building at all times. SANITIZE YOUR HANDS BEFORE ENTERING THE BUILDING: We ask that you use our hand sanitizer located at all entries immediately when entering the building and every time you reenter. TEMPERATURE CHECKS UPON ENTERING: We require every staff member, student and visitor to have their temperature taken at the door prior to walking into our building. A temp higher than 100 degrees will not be allowed into the building. CLEANING We are increasing our efforts of sanitation and cleaning over and above what we normally do. We are implementing cleaning of high touch areas before, during and after each class as well as spraying disinfectant spray on all mats and barres. Bathrooms will be disinfected frequently throughout the day as well. Purifiers are placed in each room in the building for better air circulation. DISCONTINUED UNTIL FURTHER NOTICE: All drinking fountains and vending machines NOT FOR USE. The lobby is closed, therefore all parents will need to wait in their cars. CLASS: To comply with social distancing, all instructors are required to stay at least 6 feet away from students with the exception of cheer. There are square boxes marked on the floor to mark where students are required to stand for the duration of class.EXITING THE STUDIO All students will exit the building promptly after class has ended. Please make sure to be on time for pick up or call ahead if running late. Teachers will be watching students as their rides arrive, but will also need to sanitize and get next class checked in properly. Students will need to exit the building wearing a mask. This is to help us prepare for the next class and to limit the amount of people in the studio at one time. ARRIVAL: Please be sure to register for class in advance, and please refrain from arriving more than 10 minutes before class is scheduled to begin. Please do not enter the building unless you’re approved to come in by a staff member. WAVIER: Please be sure our waiver is completed before your student attends their first class or session.ENTRANCE: Please be sure to pay attention to what room or gym your student is assigned to. Your student will need to enter through their specific door. BATHROOM:Bathrooms will only be opened to staff and students. Please try to have athletes use the bathroom before coming to class/practice.Please stay home if you’re sick. If you have symptoms of COVID-19 or have been exposed to COVID-19 please DO NOT enter our building and notify the front desk immediately. Recognize personal risk factors. According to U.S. Centers for Disease Control and Prevention (CDC), certain people, including older adults and those with underlying conditions such as heart or lung disease or diabetes, are at higher risk for developing more serious complications from COVID-19. *
Assumption of Risk At LCDC we go to great lengths to keep you informed and up to date. At any time if you have questions or concerns please see the front desk person or contact owner, Linsey Battle directly at 586-566-7055. COMMUNICATION: Monthly E-Newsletters for fall/winter sessions, email, social media and lobby communication center. Our main form of communication is through email. Please be sure your email is updated at the front desk. ATTENDANCE Regular attendance is vital to student progress and group choreography. Please make every effort to attend each class. Students may make up missed classes. If you miss more than six classes throughout the dance/cheer season you may be dismissed from participating in the recital. Also, if you miss two of the final eight classes before recital you may be dismissed from participating in the recital routine at the discretion of the teacher. PARENT OBSERVATION At this time parents are not allowed in the facility due to CDC guidelines. We cannot allow parent observation until further notice. If a parent needs to enter the building for an urgent reason please note we will take your temperature and you will need to wear a mask. INCLEMENT WEATHER POLICY In the event of inclement weather, an email will be sent and a Facebook/social media post made by 2:00pm if classes are cancelled (by 8:00am for morning classes). Please note that we do not necessarily follow school closings as roads are often cleared by the time studio classes begin. Any cancelled classes may be made up on your own time from our regular schedule of classes. In the event there is not a make up class available, a replacement class will be scheduled toward the end of the season. DANCE DRESS CODE As of 7/13/2020 MASKS TO ENTER THE BUILDING ARE REQUIRED. Required dancewear and dance shoes must be worn to all classes. Failure to wear required dancewear to class could result in students being asked to sit out the class. Repeated failure to wear required dancewear could result in termination of classes. Students are not permitted to wear jeans or shoes worn on the street or outdoors, to any dance class. For safety reason dancers are not permitted to wear jewelry. Long hair must be tied back. PARENTS RESPONSIBILITY TO BE AWARE OF DATES & EVENTS It is the responsibility of the parent or adult student to be aware of all center activities, such as viewing days, recitals, extra classes, and date the center is open or closed. The parents will give the center their email address to email all such notices home. Notices will also be placed in your family folder located in the front of the studio. It is the parent's responsibility to regularly check their emails and folders to ensure they are informed. It is the responsibility of the parents or adult students to inform the center of any email address changes, home address change or telephone number change. ✔ I have read the above and agree. Release of Liability CARE OF STUDENTS The school is not responsible for providing before or after class care for students. Parents with students under the age of 5 are to remain outside the building or in their vehicles. Students are not to be left at the center for excessive time before or after class. INJURIES Parents, legal guardians of minor students and adult students waive the right to any legal action for any injury sustained on studio property resulting from normal dance activity or any other activity conducted by the students before, during or after class time. SECURITY Security cameras are located in public areas for safety and security. PHOTO RELEASE The studio is hereby granted permission to take photographs or video of the students to use in brochures, web sites, social media, posters, advertisements and other promotional materials the studio creates. Permission is also hereby granted for the studio to copyright such photographs in its name. A lost and found basket is located in the dressing room. Please check the lost and found basket regularly as all the lost and found items are donated to Goodwill on the 1st of each month. COVID-19 NEW PLEASE READ Acknowledgment of Risk and Liability Waiver I am aware that my child will be involved in the normal hazards of physical activities. In consideration of acceptance of this application, intending to be legally bound, hereby, for ourselves, our heirs, executors and administrators waive and release all rights and claims that may arise against Linsey's Cheer & Dance Center, and any persons affiliated with this organization. I give permission to the directors of Linsey's Cheer & Dance Center to provide and approve immediate and responsible emergency care and transportation should it be required. I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance/cheer. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all rights and/or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Linsey's Cheer & Dance Center, its employees, assistants, officers and volunteers. I give permission to the staff of Linsey's Cheer & Dance to provide and approve immediate and responsible emergency care and transportation should it be required. Injury Release: It is understood that the risk of physical injury is inherent in dance training. Linsey's Cheer & Dance Center strives to reduce that risk through proper training techniques. However, by signing this form, the undersigned is willing to assume those risks and release, hold harmless, and indemnify Linsey's Cheer & Dance Center, its related entities, agents, employees, officers and representatives from and against any and all claims, demands, actions, judgments which the undersigned, or any person ever had, or may have against Linsey's Cheer & Dance Center, for any losses, costs and expenses (including attorney's fees) and damages or injuries known or unknown, real or personal, sustained by me or my child while in attendance and/or participating in all LCDC programs. The undersigned also agrees that he or she will not hold LCDC responsible for the loss or damage of personal property while in attendance and/or participating in any Linsey's Cheer & Dance Center programs or performances. COVID-19 Acknowledgement and Waiver Of Liability The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Linsey's Cheer & Dance Center (LCDC) has put in place preventative measures to reduce the spread of COVID-19; however, LCDC cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending LCDC could increase your risk and your child(ren)'s risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending LCDC and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at LCDC may result from the actions, omissions, or negligence of myself and others, including, but not limited to, BDA employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)'s attendance at LCDC or participation in LCDC programming ("Claims"). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless LCDC, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of LCDC, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any LCDC programs. By signing this, I to the best of my knowledge do not have symptoms and have not been in contact with anyone who has symptoms or has tested positive for COVID-19. If anyone is showing COVID-19 symptoms I will not send my child into the studio. I am signing this for my athlete as well. Payment Policies Tuition rates are for the full season/session ( including studio holidays), not by number of classes in a month. After registration monthly tuition must be paid by automatic bank account debit or automatic credit card payment, fall/winter tuition will be debited from your account on the 1st day of each month. For summer classes and camps the full fee is due at the first day of class. Please be aware of our 48 hour cancellation fee or your account will be charged $20 . Accounts not paid by the 10th of each month will be charged a $10 late fee. Recital costumes will be paid by automatic bank account debit or automatic credit card. A $35 deposit per costume will be due November 1st. The balance will be due January 1st. NSF on paper checks, auto-debit, or auto-charge payments will incur a $20 NSF fee which will be paid at the front desk along with the tuition. There is a two month minimum for all lessons. One-month notice from the first of the month is required to discontinue any payments. To withdraw a parent must 1. Inform the studio in person, and 2. Complete and sign a withdrawal form. Automatic bank debiting or credit card charges will stop after the one-month notice period. A valid credit card, debit card or a voided check MUST be on file throughout the season.
