APUS Tax Clinic 2024: Appointment request form

The Community Volunteer Income Tax Program (CVITP) has existed since 1971 and is a longstanding partnership between the Canada Revenue Agency (CRA) and community organizations and their volunteers. Participating organizations host free tax clinics where volunteers complete income tax and benefit returns for eligible individuals to ensure they receive, or continue to receive, the benefits and credits they are entitled to. 

The Association of Part-time Undergraduate Students at the University of Toronto (APUS) is one of the community organizations hosting a free tax clinic this year. If you are an APUS member and require help filing your 2023 taxes, please complete the following form. If you are eligible for tax clinic support, we will be in touch to offer you either an in-person or virtual appointment booking link.

What to bring to a tax clinic appointment

  • Government-issued photo ID
  • Most recent Notice of Assessment (note: only for people who previously filed taxes)
  • Social Insurance Number (SIN) 
  • T4 – Salary/wages (T4 provided by employer)
  • T4A – Scholarships, bursaries
  • T2202 – Tuition fees and education amount (from ACORN)
  • T5 – Investment income
  • RRSP contributions
  • Charitable contributions
  • Medical expenses (not covered by a medical plan)
  • Property taxes or rent paid (rent cheques or a letter from your landlord)
  • For international students, the date of arrival to Canada, and immigration status (if a new immigrant)
  • For parents, a list of your dependents, exact dates of birth, and SINs, spouse, partner info
