This form is to be completed by a legal parent or guardian of the participating child. It can be used to sign up the same child for multiple sessions, but a separate registration form must be completed for each child.

PLEASE NOTE: FDC will follow all masking and COVID recommendations set by the CDC and local state, county, city, and federal governments.  

Your registration is not complete until this form is submitted and payment is received. 5 day camps are $350 and 4 day Camps are $280.
