This page covers information about your background and is required of all applicants.  Subsequent pages request information for the specific positions (Teaching UGTA, Dissecting Intern) you are interested in.  If accepted for multiple positions, you may accept any combination of them.

How many credits have you already earned in IPHY 3010 (Teaching in IPHY)?  NOTE: You are limited to 6 credits, and these UGTA positions will award 2 credits.  If you are selected for a UGTA position, but subsequent staff review of your record shows that your cumulative IPHY 3010 credits would exceed 6 as a result, then you will not be allowed to assume the position.
UGTA Experience *
How many times have you been a UGTA for human anatomy lab (either 3415 or 3417)?  NOTE - If you have UGTA'd 3 times, then you are only eligible for dissecting internships.

If your answer to the previous question was zero, please write "N/A".  Otherwise, for each semester you have been a UGTA in the human anatomy lab, please indicate the semester, year, and the name of the graduate TA you worked with.  (If you were a  Cadaver Specialist UGTA then indicate that, and include the name of the lead dissector you worked with on the cadaver.)
