2020-21 Universal Flexibility Decision Form
Please complete the following form to capture your school's SY2020-21 Universal Flexibility decisions. Be sure to reference the handbook and other support documentation on the Universal Flexibility website on the DPS Commons (http://thecommons.dpsk12.org/universalflexibility) for guidance while making your decisions. If you have questions please reach out to flexibility@dpsk12.org.

Any 'decline' decisions will require a Confirmation and Planning Meeting with the pertinent department and RIS/RAIS or ED to ensure replacement choices meet District and State legal and technological requirements. If you decline a curricular or assessment recommendation, this form will ask you to identify your replacement vendor or replacement assessment strategy.

This survey may not save so please set aside ~15 minutes to complete the form.

This form will close at 5pm on February 6, 2020. All decisions are final as of that date.
