KNOWLEDGE OF SPORT AND DISCIPLINE

I have taken every reasonable step to familiarize with and educate myself about the inline and roller skating discipline(s) I will be involved and/ or participating in, including knowing and understanding the sport rules (technical rules, how the sport happens), required safety equipment, related and anticipated risks, the culture and norms related to the discipline(s), knowing what equipment and clothing is required, familiarizing myself with the field of play (venue/ track/ arena/ road/ outside or inside) in which the discipline is held, and have an awareness for my level of performance and fitness, and skills relative to others in the discipline I will be involved and/ or participating in.

ETHICS
I acknowledge that my registration with Roller Sports Canada, henceforth known as RSC, can be revoked or cancelled as a result of my failure to adhere to ethical standards which are opposition to racism, sexism, hetero-sexism, unsafe behaviour, discrimination, bullying, criminal conduct, behaviour deemed by event officials (judges, referees) to be dangerous and/ or of an extreme negative nature and intent, and harassment. Further actions (removal from team, club, sponsor, ban) can be put into effect.

ANTI-DOPING
I acknowledge that the anti-doping bodies (such as World Anti-Doping Agency (WADA), Canadian Center for Ethics in Sport (CCES)) have easily accessible resources and lists outlining prohibited and controlled substances and methods. Failure to respect the rules and regulations and anti-doping policies and protocols (for example using prohibited methods or prohibited substances) can lead to the revocation or cancelled of my RSC registration. Further actions (removal from team, club, sponsor, league, event registration, ban) can be put into effect.

RISKS
I acknowledge that the inline and roller skating and all their related disciplines can involve a test, at times extreme, of a person’s physical and mental limits and carries the potential for death, serious injury and property loss. There are both known and unanticipated risks. The known risks include, but are not limited to, those caused by the challenges inherent in social, recreational, fitness, and competitive athletic training, activities and events, by terrain, facilities, temperature, weather, the physical and mental condition of participants including my own physical and mental condition, my acts or omissions or failure to act, first aid, emergency treatment or other services rendered, consumption of food or drink, lack of hydration, latent or apparent defects or conditions in any equipment or property supplied by Roller Sports Canada (RSC) or other persons and/or entities, vehicular traffic and actions of other people including, but not limited to, participants, volunteers, spectators, coaches, event officials, team officials, league officials, event monitors, and/or organizers of activities/ events/ games/ matches/ training/ practice/ club/ team/ league. I understand and acknowledge that the above listing is not complete or exhaustive, and that other risks, known or unknown, identified or unidentified, anticipated or unanticipated, foreseeable or unforeseeable, may also result in injury, or damage to myself or property, and I expressly accept and acknowledge those risks not specifically listed above as well. I expressly accept and acknowledge that RSC related activities also involve the risk of severe economic and property loss and damage. These risks are not only inherent to participants, but are also present for volunteers, officials, coaches, spectators, managers, coaches, judges. Being aware that engaging in, participating in, or observing RSC related activities/ events/ games/ matches/ training/ practice/ club/ team/ league entails the risk of injury to myself and the risk of injury to participants, spectators, or other third parties, I expressly agree, covenant, and promise to accept and assume all responsibility and risk for injury, death, illness or disease, damage to myself or to property, to participants, spectators or to other third parties and their property, arising from or relating to my participation in or observing RSC related activities/ events/ games/ matches/ training/ practice/ club/ team/ league. I am voluntarily electing to participate in, attend, and/or observe RSC related activities/ events/ games/ matches/ training/ practice/ club/ team/ league in spite of these risks.

ASSUMING RISKS
I hereby assume all of the risks of participating and/or volunteering in RSC related activities/ events/ games/ matches/ training/ practice/ club/ team/ league. I realize that liability may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained or controlled by them or because of their possible liability without fault.

OBSERVING UNSAFE BEHAVIOUR
I hereby agree that if at any time I feel any RSC related activities/ events/ games/ matches/ training/ practice/ club/ team/ league to be unsafe or if I observe unsafe behavior on the part of other participants/observers, I will immediately notify the appropriate RSC officials and refuse to participate in any further activities or events until all unsafe conditions observed by me have been remedied.

CERTIFYING THAT I CAN PARTICIPATE
I certify that I am physically fit, have sufficiently trained for participation in RSC related activities/ events/ games/ matches/ training/ practice/ club/ team/ league and have not been advised otherwise by a qualified medical person. In the event that I am advised by medical authorities not to participate in RSC related activities/ events/ games/ matches/ training/ practice/ club/ team/ league or I become injured, ill or physically unfit to participate in same, I undertake not to participate until I become physically fit.

WAIVER
In consideration of my application for registration with RSC and permitting me to participate in in RSC related activities/ events/ games/ matches/ training/ practice/ club/ team/ league, I hereby take action for myself, my personal representatives, legal representatives estate trustees, executors, administrators, guardians, heirs, distributees, legatees, next-of-kin, successors, and assigns as follows:

(A) Irrevocably waive, release and forever discharge from any and every claim, demand, action or right of action of whatever nature or kind, either in law or in equity, and all liability for my death, disability, disease, personal injury, property damage, property theft or actions of any kind which may hereafter accrue to me in connection with my participating in, or my traveling to and from, RSC related activities/ events/ games/ matches/ training/ practice/ club/ team/ league, RSC, their past, present and future directors, officers, members, employees, servants, volunteers, representatives, and agents, event holders, event sponsors, event directors, event volunteers, and event officials, (collectively the “Releasees”)

(B) Indemnify and Hold Harmless the Releasees for myself, my personal representatives, estate trustees, executors, administrators, heirs, legatees, next-of-kin, successors, and assigns (collectively “Successors”) I HEREBY WAIVE, RELEASE, DISCHARGE, HOLD HARMLESS AND PROMISE TO INDEMNIFY AND NOT TO SUE the Releasees and the sponsors of RSC related activities/ events/ games/ matches/ training/ practice/ club/ team/ league, the organizers and any promoting organizations, property owners, law enforcement agencies, all public entities, special districts and properties, and their respective agents, officials, and employees through or by which practices and/or events will be held, (the foregoing are also collectively deemed to be Releasees) FROM ANY AND ALL RIGHTS AND CLAIMS INCLUDING CLAIMS ARISING FROM THE RELEASEE’S OWN NEGLIGENCE which I have or which may hereafter accrue to me and from any and all damages which may be sustained by me directly or indirectly in connection with, or arising out of my participation in or association with RSC related activities/ events/ games/ matches/ training/ practice/ club/ team/ league or travel to or return from RSC related activities/ events/ games/ matches/ training/ practice/ club/ team/ league.

I hereby consent to receive medical treatment, which may be deemed advisable in the event of injury, accident and/or illness during any RSC related activities/ events/ games/ matches/ training/ practice/ club/ team/ league. In connection with any injury that I may sustain or other medical conditions I may experience during my participation in or attendance at any RSC related activities/ events/ games/ matches/ training/ practice/ club/ team/ league, I authorize any emergency first aid, medication, medical treatment or surgery deemed necessary by the attending medical personnel if I am not able to act on my own behalf. I further authorize the attending medical personnel to execute on my behalf any permission forms, consents, or other appropriate documents relating to medical attention and to act on my behalf if I am not able or immediately available to do so.

I expressly agree that this Acknowledgment, Consent, Undertaking, Release, Waiver and Indemnity is intended to be as broad and inclusive as permitted by the laws of the Province shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law and that if any portion thereof is held invalid, I agree that the balance shall, notwithstanding, continue in full force and effect.

CONCUSSION
I will help prevent concussions by:
• Wearing the proper equipment for my sport and wearing it correctly.
• Developing my skills and strength so that I can participate to the best of my ability.
• Respecting the rules of my sport or activity.
• My commitment to fair play and respect for all* (respecting other athletes, coaches, team trainers and officials).

I will care for my health and safety by taking concussions seriously, and I understand that:
• A concussion is a brain injury that can have both short and long-term effects.
• A blow to my head, face or neck, or a blow to the body that causes the brain to move around inside the skull may cause a concussion.
• I don’t need to lose consciousness to have had a concussion.
• I have a commitment to concussion recognition and reporting, including self-reporting of possible concussion and reporting to a designated person when and individual suspects that another individual may have sustained a concussion.* (Meaning: If I think I might have a concussion I should stop participating in further training, practice or competition immediately, or tell an adult if I think another athlete has a concussion).
• Continuing to participate in further training, practice or competition with a possible concussion increases my risk of more severe, longer lasting symptoms, and increases my risk of other injuries.
I will not hide concussion symptoms. I will speak up for myself and others.
• I will not hide my symptoms. I will tell a coach, official, team trainer, parent or another adult I trust if I experience any symptoms of concussion.
• If someone else tells me about concussion symptoms, or I see signs they might have a concussion, I will tell a coach, official, team trainer, parent or another adult I trust so they can help.
• I understand that if I have a suspected concussion, I will be removed from sport and that I will not be able to return to training, practice or competition until I undergo a medical assessment by a medical doctor or nurse practitioner and have been medically cleared to return to training, practice or competition.
• I have a commitment to sharing any pertinent information regarding incidents of removal from sport with the athlete’s school and any other sport organization with which the athlete has registered* (Meaning: If I am diagnosed with a concussion, I understand that letting all of my other coaches and teachers know about my injury will help them support me while I recover.)
I will take the time I need to recover, because it is important for my health. • I understand my commitment to supporting the return-to-sport process* (I will have to follow my sport organization’s Return-to-Sport Protocol).
• I understand I will have to be medically cleared by a medical doctor or nurse practitioner before returning to training, practice or competition.
• I will respect my coaches, team trainers, parents, health-care professionals, and medical doctors and nurse practitioners, regarding my health and safety.

PHOTO RELEASE AGREEMENT

I hereby grant Roller Sports Canada and it's instructors permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.

I understand and agree that all photos will become the property of RSC. and will not be returned.

I hereby irrevocably authorize RSC. to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

I hereby hold harmless, release, and forever discharge RSC. from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I CONFIRM THAT I HAVE READ AND UNDERSTOOD THIS RELEASE AGREEMENT PRIOR TO SIGNING IT, AND I AM AWARE THAT BY SIGNING THIS RELEASE AGREEMENT I AM WAIVING CERTAIN LEGAL RIGHTS WHICH I OR MY HEIRS, NEXT OF KIN, EXECUTORS, ADMINISTRATORS, ASSIGNS AND REPRESENTATIVES MAY HAVE AGAINST THE RELEASEES.

Electronic Signature Disclosure and Consent

By selecting the “I agree” button, I am signing this document electronically. I agree that my electronic signature is the legal equivalent of my manual/handwritten signature on this document. I am also confirming that I am authorized to enter into this Agreement. If I am signing this document on behalf of a minor, I represent and warrant that I am the minor’s parent or legal guardian.

I may decline to electronically sign this document and withdraw my consent to sign this document electronically by contacting the signature requestor directly, which may delay transactions. I may contact the signature requestor separately to request to sign this document on paper or to receive a paper copy of the signed document. Any fees for such paper copy will be charged then by the signature requestor. When I sign the document, I will receive a copy of this document via email.

I understand that the electronically stored copy of my signature, any written instruction or authorization, and any other document provided to me is considered to be the true, accurate, and complete record, legally enforceable in any proceeding to the same extent as if such documents were originally generated and maintained in printed form. I agree not to contest the admissibility or enforceability of the electronically stored copy of this document and any other documents. I agree to the terms and conditions of this document on behalf of myself or as the parent or legal guardian of the minor on whose behalf I am signing this document.
