We reserve the right to dismiss any athlete whose conduct is detrimental to the overall good of the clinic.  In case of gross misconduct, no refund will be made.  No deduction is allowed for late arrival or early departure.  I hereby authorize the director of NMGVFC to act according to their best judgment in any emergency requiring medical attention. *

Prior to participation, payment (checks or Money Order payable to NMGVFC or cash) signed concussion paperwork, and a copy of insurance billing information must be sent to NMGVFC, 4645 Dufek Rd, Lake Leelanau, MI  49653 or brought with athlete to the first session. *
