If you are registering a new student who hasn't taken lessons through the Academy before, this form is for you!

Please fill out a form for each student taking lessons.

A new student registration fee of $20 is required after a lesson time is agreed upon. This will reserve your spot. 
This is for NEW students only and is a one-time fee.
