All holiday workshop fees are to be paid for to secure your place. All fees are non refundable, regardless of absence. I allow my child to leave the premises with the group and walk to the nearby park for lunch. I acknowledge and have stated on this form any existing injuries and medical conditions that the student is suffering from, and take sole responsibility for seeking medical clearance before participating in any Elevate Performing Arts activity. I acknowledge that Elevate Performing Arts does not take any responsibility for injuries sustained during or following any Elevate Performing Arts activity. I acknowledge that some classes may be filmed / photographed and that this footage / these images remain the property of Elevate Performing Arts to be used for marketing purposes. If you are more than 10 minutes late to pick up your child after 3:00pm, you will be charged for aftercare. You will be required to pay $15 per half hour you are late.
