This form is to be completed by any adult/coach/committee member (aged 18 years or older) wishing to join or be involved in Wexford Hockey Club during the 2021-2022 season.

Wexford Hockey Club strives to safeguard all our members. In an effort to do so we are asking all our adult members to complete this registration & code of ethics form. The Club is asking you to do this so we have the most up to date contact information for you, have an emergency contact person for you should we need to contact them on your behalf during training/the field of play or if you need medical attention and to make you aware of your responsibilities as a member of the Club. The information you provide will only be used for hockey related matters.
