Head coaches please use this survey to rate each of your player's for all the criteria listed below. This information:

1) Helps us identify strengths and areas that need improvement.
2) Gives us the opportunity to group players by skill levels.
3) Creates a climate to achieve individual improvement through specific targets.
4) Provides information necessary to make objective assessments of the performance level of each player.
5) Assures that development is a responsibility shared by the entire organization.

Please note: The players or parents will NOT see this form and they will NOT receive summary data from this survey. Your evaluation will be used during the team formation process that your age director and WAC board will participate in after try-outs.
