Jaguar Cup Entry Form
Thank you for choosing to enter our development tournament! This is to introduce students to the rules and spirit of tournament, while also allowing us to develop our referees and table officials. 

Cost: 
R250 for one division
R280 for kata and kumite 
R300 for kata, kumite and unison kata

Venue: Arthur Matthews Primary School, Florida
Time: 8:00am to 17:00 (detailed schedule will be released closer to the time) 
Rough schedule: 
8am - 11am - white and yellows
11:30 - 1:30: oranges and greens 
2:00 - 5:00pm: Blues and up, plus all adults of all ranks.

Dress: Full karate suit. Gumguard recommended for kumite. 
The dojo will provide mitts, and shinners and footers for older students. 

ENTRIES WILL CLOSE MONDAY THE 20TH OF MAY AT 9AM. 
In order to ensure the smooth running of the tournament, no late entries will be allowed, and especially not on the day.

We/I indemnify Goju Ryu Karate Centre, OGKK Gauteng, its instructors and assistants, against any accident, injury or loss caused in any way whatsoever or cost incurred on account of such injury or loss that may occur as result of participating in the Jaguar Cup. We know of no prior health issues or current injuries that might compromise participation in this event.

If I/my child is ill, I will notify my instructor and withdraw my entry. 

I acknowledge that I will reserve a space once this form is submitted, and I will pay into my dojo's account to confirm my space.
