  • This form serves as a Slot Booking for your VTC on our Convoy written in the title of this form.

  • Your request must contain true information and everything necessary in this form, if you do not provide true information or fill in what we require, your request for a slot will be rejected.

  • The Management of our VTC that processes these requests cannot give you any information about how your request turned out or which member of our VTC is in charge of the request, this information is confidential and must not be disclosed.

  • The Management of our VTC has the right to cancel your application at any time without giving a reason, or if you do not meet any of our requirements or cease to meet them.
  • Furthermore, we reserve the right to move you from your slot to another at any time - but this information will be communicated to you.

  • This Slot Booking Request can only be completed by the management of the VTC or their Event Team, this request cannot be completed by VTC Drivers or Staff below the required role for this Slot Booking Request.

  • Requirements for Slot Booking are listed in the form.

  • We are not responsible for incorrectly completed Slot Booking Requests.

  • In case of any changes to your VTC such as name or similar, we would like you to contact us in time so that we can change this information.
