Under the provisions of the Privacy and Personal Information Act 1998 (NSW), and the Health Records Information Privacy Act 2002 (NSW) you are advised that the information contained in this application will be collected by the NSW Department of Education. This information will be used for the following purposes:
- to assess your application
- for program administration and evaluation purposes
- to offer you employment opportunities in the future
- If you require emergency medical attention, the details of your doctor may be shared with healthcare staff
- If you cause damage to accommodation, your contact details may be shared with the accommodation provider in line with section 3 of the program guidelines.
The provision of this information by you is voluntary and is not required by law, however, if you do not provide the information marked 'required' your application cannot be progressed.
