By completing this form, you acknowledge the risk of picking up trash and potentially hazardous materials. You are participating in this event at your own risk and the City of Wilmington, Plastic Ocean Project, or Affiliates of the Greenfield Skate & Sweep are not liable for any bodily injury.

You acknowledge that this Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity in which you are participating, and that it will govern your actions and responsibilities at said activity.

If under the age of 18, volunteers must be supervised by one or more adults 21 years of age or older.

Volunteers Must Not:
• Use or possess illegal drugs or alcohol during a cleanup.
• Pick up trash on bridges, on overpasses or in tunnels.
• Pick up trash on a median.
• Pick up what appears to be hazardous material or any type of container with unknown contents.
• Attempt to squeeze bags to make room for more trash (injuries from broken or jagged objects can occur).
